The normal courseof providing care means that such records may be left in administrative areassuch as the front office, etc. You agree to the normal procedures utilized withinour facilities for the handling of charts, patient records, PHI, and otherdocuments or information.

 

·     We utilize a number of vendors in the conduct of our business. Thesevendors may have access to PHI but agree to abide by the confidentiality rulesof HIPAA. In all these cases, we will have a HIPAA business associate contract inplace with our vendors, as applicable. This means they understand the federalHIPAA guidelines for confidentiality and agree to abide by those regulationsset forth and maintain the same level of confidentiality that healthcareprofessionals are bound to in the event they should encounter patientinformation. Careful steps will be taken with our accountant and IT vendor toensure they rarely encounter any client information. Our Electronic MedicalRecord Systems (EMR) vendor will be used tomaintain patient records. In both cases, bank level security and encryption isused to protect client information. Their systems will also electronicallyautomated and vendor support reps are only accessed if a data input error occurred.Support staff of the EMR vendor will have restricted access and will not beable to access patient narrative notes. Our EMR vendor, like most EMR vendors,will have authority to use de-identified patient information. They will do thisin compliance with HIPAA guidelines to ensure any data extracted for researchpurposes can in no way be identified to a client.

 

·     You understand and agree to inspections of our locations and the reviewof documents, which may include PHI by government agencies or insurancecompanies in the normal performance of their duties.

 

·     You agree to bring any concerns or complaints regarding privacy to ourattention.

 

·     Your confidential information will not be used for the purposes ofadvertising or marketing of products, goods, or services. Such prohibition doesnot include treatment/product samples or goods of nominal value.

 

·     We agree to provide you with access to your records in accordance withstate law. We may change, add, delete, or modify any of these provisions tobetter serve the needs of both you and our practice. You hereby agree to theterms set forth above and any subsequent changes in LIV’s policy and understandthat this consent shall remain in force so long as you are using any of theServices.
